Discharge Planners and Social Workers have a challenging job requiring the balancing of multiple responsibilities that at times may be at cross-purposes. At Central Coast Senior Services, Inc, we understand the difficulties of your day-to-day obligations. Our Short Term Care After Hospitalization or Surgery Program provides a few days of short-term supervised home care, with the goal of creating safer home environments and fewer re-hospitalizations. We offer a variety of options at very discounted rates.

When you make a referral to Central Coast Senior Services, Inc, one of our Home Care Coordinators can be in your facility very quickly, usually within hours. They will interview and provide information to your patient,  and follow-through with a home safety assessment to make sure your patient is discharged into safe circumstances, helping to ensure that your Discharge Plan is implemented fully in the home.

Our Home Care Coordinators will also collaborate closely with the services of licensed Home Health Agencies with the specific goal of improved post-hospitalization outcomes and fewer early readmissions. We also facilitate the transition of your patients from your facility to Home Hospice, providing that extra measure of personal contact that can ease the anxieties of your patients and families.
